What should I do about this situation? Please Help?

Last year I violated my probation for not paying the probation fee's or any restitution. I have been under psychiatric care for well over 3 years with a history of Major Depression, Bi-Polar, Psychosis, ADHD and a few other things like several suicide attempts. My Psychiatrist at the time was very good with me spent 45 minutes with me each session to talk to me and also do meds which is uncommon for most Psychiatrist who only see you for 10 mins fill out scripts and send you on your way. Well my Psychiatrist at the time wrote me a letter for court that was very detailed in listing all of my diagnosis's and he even listed my meds I was taken for each problem and what they were for which is like 10 different medications. That letter helped me tremendously in court because the judge read the letter and a minute later said case dismissed we are going to terminate his probation and let him go. Now this year I have a new case pending and a new Psychiatrist because my old one retired and this guy and his secretary tell me they will not write any such letter with any information on it for me. They just flat out refuse. Is there anything I can do to make him write me a letter? I mean all I'm asking for is a simple letter that states "To Whom It May Concern...... list my diagnosis's and med list stating what each medication is for." What is the harm in that? Why is my Psychiatrist being such a butt whole? And for the record I'm not some serious criminal I really do have a lot of medical issues that cause me to someones black out and be impulsive.
